Care Management Works
Personal Assessment
ElderTree will conduct a personal assessment with the family
and the elder. Interview questions cover a range of topics relevant to your
elder's health and living situation, including everyday activities, nutritional
status, safety, memory, depression, finances, and more.
Developing
a Care Plan
A Care Plan includes the results of the assessment, recommendations,
and referrals for local care resources(Elder law Attorneys, Home Care Agencies,
Adult day Care Providers, Assisted Living Facilities, etc.). ElderTree will
go into great depth in explaining the details of the plan, what led to each
recommendation, what the next steps are, and what you can expect.
Arranging
Services
ElderTree helps to arrange and monitor services to meet the
individual needs of your loved one. ElderTree is uniquely connected in the
community. We know many of the eldercare services and their reputations, and
can help ensure success by working with others whose history and strengths
we understand.
Ongoing
Care Management and Communication
Our first meeting establishes a foundation for short and long-term goals.
Follow-up visits will be compared to that initial assessment to determine
what adjustments need to me made. Through weekly or bi-weekly visits, we can
identify issues early and address them.
