ElderTree can bring you peace of mind by helping you prioritize through our
assessment and care plan. An ElderTree Care Manager will come to your elder’s
home and conduct a comprehensive assessment. Through the assessment we will
write a care plan which will include short and long-term solutions, as well
as community resources to meet your unique needs.
Through weekly or bi-weekly visits we can oversee the big picture and provide
on-going care coordination, monitoring and advocacy. We even provide emergency
on-call services should the need arise.
Remaining
at Home: Safety Consultation
ElderTree offers a customized Memory Impairment Safety Assessment (MISA) of
the home environment. We will provide you with written recommendations and
valuable community resources. ElderTree can assist you in implementing the
Safety Plan.

the Best Long-Term Care Environment: Transition Services
ElderTrees expert insight helps you choose the right long-term care setting
for your loved one. It is vital to find a community that is dedicated to caring
for people with memory loss in a way that nurtures the human spirit as well
as provides superior healthcare. ElderTree will walk with you through the
decision making process to choose the best setting. Once the elder has moved
we offer Transition Care Management Services in which we help the elder adjust
to their new home through weekly visits and ongoing consultation.
Activity
Enrichment Program
ElderTree provides an Activity Enrichment program in which a Professional
Geriatric Care Manager will visit with your loved one on a weekly or bi-weekly
basis. The Care Manager, specifically trained in activities for memory impairment,
will spend one on one time with the elder and provide the staff and family
with ways to encourage and support participation in meaningful activity.
