About The Program

Whether you live at a distance while caring for your parents, or you are local and need support in managing your loved one’s care, our program is the solution for you. Our Assisted Living at Home program provides comprehensive coordination of care, with the appropriate oversight needed to ensure an optimal quality of life for loved ones living at home. With over 18 years of experience providing care management in the home, it was evident that a comprehensive program to arrange Assisted Living at Home is widely needed. With the addition of COVID-19, this program has become the complete solution for our clients and their families.

On Call Services

We are on call for your loved one around the clock! When a care question arises, or there is an urgent emergency room visit, our ElderTree care manager will be there for you, as an advocate to ensure the next best steps. ElderTree is much like an executive director of an assisted living community. We coordinate all aspects of safety, health, and well-being while providing an ongoing dialog and collaboration with the adult children, family, and friends.

These five pillars of support are needed to provide the best possible outcome when planning and managing care at home. With our expertise and our experience in the field of aging, we have created a holistic approach that places the older adult at the center of the program using the support of technology to age in place successfully.

Quality of Life

ElderTree uses a whole person-centered care model to provide the richest quality of life possible for all of our clients. This means that we focus on your hopes and dreams, strengths and potential to help you live your “best life.” 

We insist on including the input of the older adult in all decisions about their lives. “Nothing about me without me” is our guiding principle. Using our thorough life history, engagement, and medical assessments we are able to design a plan that brings meaning and purpose to each day.

We have researched the latest in technology to promote engagement and socialization and will include technology alongside quality person-to-person relationships when appropriate.

Wellness

ElderTree manages all doctors’ appointments and ensures communication and coordination between healthcare professionals.

We are on-call around the clock and act as your advocate should there be an unexpected emergency room visit. We ensure that your medication is organized and that necessary medical supplies are on hand.

ElderTree has invested in a care management software system to keep real time medical records. We will implement medication dispensing and monitoring technology and other forms of wellness technology solutions as needed.

Care Team

ElderTree is the Executive Director of your care team and will bring other professionals onto the team as necessary to meet your needs.

This could include, although is not limited to, a home care agency (provides caregivers), a home health agency (provides therapies covered by Medicare), a money manager (assists with paying monthly bills), a handyman (to install safety equipment, home modifications, etc), a traveling physician, a dietitian, a pharmacy, and more.

We will facilitate communication between all members of the care team and make sure that all members of the team are providing excellent service.

Plan of Care

ElderTree develops a person centered plan of care as a road map to keep everyone informed of the goals that pertain to all of the pillars of support and a clear timeline when the goals will be met. This is a working document created and maintained by your primary Care Manager.

The care plan includes assessments and updates on your physical health, mental health (including cognitive functioning and memory), and additionally addresses safety, emergency planning, legal documents, nutrition, social engagement, spiritual needs, recreation, and more.

Home Safety

ElderTree will use our home safety checklist to make the home as safe as possible and reduce fall risk.  We will recommend and install life alert systems and or movement monitoring systems depending on your specific needs. 

We will gather all of the emergency paperwork and legal documents in a binder located in your home. Our care manager will keep the binder updated in the event that an emergency should occur. We will also discuss your disaster preparedness plan and make this all a part of your plan of care.
