Our Process

Consultation

Our care manager will meet with the older adult and family in their home to listen to their specific concerns and challenges.

Drawing on our extensive experience in eldercare and our knowledge of local providers, we will present available resources and craft a personalized solution that fits your loved one’s needs.

Ongoing Management

Families and their elders often decide to utilize our team to coordinate the solutions identified in the consultation. We designate one primary care manager to each of our clients so that we can establish an ongoing relationship with your loved one. Our ongoing care management services focus on three areas:

Consulting

We help families understand the challenges facing their aging family members.

Working with families and their elders, we help facilitate difficult discussions such as driving, independent living safety versus quality of life concerns, and crisis intervention.

We also help families identify eldercare specialists for their legal, financial, and medical questions.

Finally, families who live at a distance from their loved ones count on us to be their eyes and ears to make sure things are going well and to alert them before problems develop.

Advocacy

Our seasoned care managers are advocates for our clients.

By visiting with our clients, we can monitor the quality of the care they are receiving at home or in a community and advocate on their behalf.

We can also accompany your loved one on medical appointments and visit during hospital stays, serving as their advocate with medical professionals.

We are on call around the clock in case a care need should arise or an urgent trip to the emergency room is required, and a care manager is needed to advocate on the clients behalf.

Care Coordination

Serving North Virginia and the Washington, D.C. metro area since 2004, we have long standing professional relationships with high quality service providers in the area.

We can coordinate the following services:

  • Assisted living at home

  • Available on call 24/7 

  • Medical care and appointments

  • Emergency room/hospital advocacy

  • Choosing and moving to retirement community, assisted care facility, or nursing home

  • Life enrichment visits

If your loved one has an urgent matter, ElderTree can help. We have a care manager on call and available 24 hours a day, seven days a week: if our clients face an emergency room visit they don’t have to face it alone.
